Event: Concert at St. Thomas the Apostle Hollywood

Choirs: Vocal Ensemble & Advanced Women's Chorale

Location:  St. Thomas the Apostle Hollywood, 7501 Hollywood Blvd., Los Angeles, CA 90046

1:30 pm: students and chaperones to meet in the Mira Costa upper Peck parking lot, bus to arrive in upper Peck parking lot
1:45 pm: scheduled departure time from school
3:00 pm: rehearsal at the church
5:00 pm: concert start time

6:30 pm: concert end time (approximate)

7:30 pm: expected to arrive back at school

Students should plan to arrive at school dressed in their performance attire.

​

All parents are invited to attend at no charge, but due to limited seating on the bus, will need to drive separately (except for chaperones, who will ride the bus with the students). Because the concert is occurring in the late afternoon/early evening, everyone riding the bus should plan to bring a bagged snack to eat on the ride back and will need to eat dinner after they get home.  

 

Attendees need to complete and return these forms before the trip:

- Field Trip Permission Form (complete and return page 4 only -- other pages not needed for this trip)

- Student Field Trip Code of Conduct

 

Chaperones:

- Chaperone Guidelines MBUSD

- Chaperone Volunteer Hold Harmless Form

 

In addition, for liability reasons, any parent driving separately, whether transporting students or not, will need to complete these forms, as well:

- Chaperone Driver Insurance Agreement

- Volunteer Driver Agreement
